Zehn Jahre nach den Ereignissen der ersten drei "Hatchet"-Filme will Andrew Yong der den Bayou Butcher überlebt hat, seine Geschichte zu Geld machen. Doch seine Biografie verkauft sich trotz vieler Talkshow-Auftritte nicht wirklich gut. Viele glauben, dass Yong selbst der Mörder ist und Victor Crowley nur eine Legende, ein Märchen ist, das er sich ausgedacht hat, um seine Taten zu verschleiern. Für einen PR-Stunt reist Yong schließlich an den Ort des grausamen Geschehens zurück – allerdings zu einem denkbar ungünstigen Zeitpunkt. Denn Victor Crowley wurde kurz vorher durch ein YouTube-Video von den Toten zurückgeholt. Als Yong und die ihn begleitende TV-Crew im Sumpfgebiet abstürzen, müssen sie daher bald ums Überleben kämpfen. Denn Victor Crowley denkt nicht daran, eines seiner Opfer entkommen zu lassen...
